JTAlert should always be in-sync with DXKeeper as they use the same data source, the DXKeeper log file. However, JTAlert is likely applying a different criteria for B4

Thanks for the offer, but I am not a fan of adding individual callsign exceptions to a database, its a maintenance nightmare, you have enough on your plate..

Have you sent Laurie a "Contact Support" request from the HELP menu in JT-Alert? This sends Laurie needed information so he can determine what is happening.
That said, what do you expect to see?

what criteria do you have set for a confirmed grid in Scan Log and Update. For example if you have required a LoTW confirmation then grids are only removed from the needed list

I am running all the DXLab programs, WSJT-X, and JTAlert on vy fast w10 64b computer, all current versions.
My understanding is that when running JTAlert, using DXK for logging, that all B4 lookups

Yes...it does require the XML subscription. So there are likely quite a few ops that don't have that.
It would better IMHO if the callsign database did it and then at least the callsign database

I see that all these callsign show correctly as USA on their QRZ lookups.
Perhaps you could just do the QRZ xml lookup to update call signs?
So use the fcc database to find the prefixes and do the QRZ
